1st Week Of Practical-HTAR-Nov2011 (MRI)

Monday - Friday
MRI


  • Went to HTAR X-ray department. (5 of us Myself,Nabil,Farha,Fatin,Shima)
  • Self-Report to HOD Madam Pushpa. 
  • She took us to "bilik seminar" and briefly explain the department rule. She ask us to complete some form in order for us to receive an access card. Basically we use it to record our attendance. 
  • Then she guide us to each x-ray dept and introduced us to radiographer on duty. Somehow A/E dept and Main dept (IDK-the name of this dept) located nearly to each other but for ACC (Kompleks Rawatan Harian) is located quite far from the other two. 
  •  We haven't got our rooster yet so she separate & assigned each of us to different room. For this week I was assigned to MRI room.
  • Went to MRI room and introduced myself to Puan NurulAin. Senior Radiographer usually are the one in charge of handling MRI. There are 50+ radiographer in HTAR if i'm not mistaken but ONLY 7 radiographer are given the authorities of doing MRI. Encik Jeffry, Encik Yani, Encik Azman, Puan NurulAin,Puan Balkis and 2 others whom i didn't have the opportunity to work with.
  • I learn how to interview patient properly (comfirm name,date,if the pt got any allergy, take medication if he/she got allergy,have done any operation,what type of operation,fasting,when was the last time he/she eat, married/single, if married is she pregnant or not) 
  • Register patient (some signature here and there,prepare folder for the patient,name,id,procedure done,from where,race,gender,) 
  • Patient positioning (depend on the procedure done, usually all the same. supine, head first, unless the pt do MRI or LL procedure, feet first), 
  • Coil used in MRI -philips- (Sense Head coil for pituitary,brain, body coil for breast,Spine coil for Spine). There are all together 10 coil. i will list all the coil after i retrieved my note next week. *left it in the MRI room. :)
  • How to prepare contrast (syringe 10 & 5ml, 10 for contrast, 5 for normal saline)
  • What contrast was use (magnevist +dextrose?) 
  • When to inject contrast (if dynamic injection needed we need to w8 for the signal from the radiographer who stay at the back of the glass window, if not, just open the door,inject the contrast then close the door. heh)
  • How to pull out branula (put some catton,salotape it at the site of injection,carefully pull it, if not properly done the blood just flow like a river wew (happen once) heh) 
  • protocol for MRI Brain,Spine and knee. (usually everything start with survey, reference, then start T1,T2,FLAIR,STIR, each have their own meaning. it is something we need to know. not really MUST know. additional knowledge.*
Regarding clinical study
  • On my first day (Monday) i did ask HOD Madam Pushpa on how can i obtain the list of common pathology admitted by patient in HTAR. Well as i expected it is not easy for us to carry out our work without having a proper Letter. Confidential issue and some ethic issue. Well Madam Pushpa is Busy on the first day so she told me to come see her on thursday.
  • On tuesday i met Encik Azman, One of the radiographer who usually in charge of being supervisor for student, well having a quick talk with him is valuable enough, he told me some of the common thing patient here being admitted to, undescended testes?? :D. Nabil immediately want to make it as his subject because his relative happen to have this kind of problem. Then He told us to go to Record Room at ACC to look for further information. So me,Nabil and fatin went there hoping to obtain something. But as i expected. We still need a proper LETTER to look through all the patient record there.
  • So on Thursday i met with Madam Pushpa again. Well this time she's a little bit confuse like us on how we are going to proceed doing our work. Because it's the same for her. she told us we need a proper letter. Then she said she's going to talk to our lecturer on this Saturday (madam Ann). So i said okay and now we can only wait for the next week to see how things proceed. :)
